Using your own secure password, you can log into the On-Line Patient Portal 24 hours a day, 7 days a week from the comfort and privacy of your home, office, or mobile device.

- Communicate with your doctor by sending and receiving secure messages.
From this Sign In page you will be able to log into your Patient Portal using the email on file, the primary phone number as listed with your provider, and your date of birth. Click the “Sign up today” link on the right side of the page. Once your account is activated, you will be able to access your account with your email and password you create.
Once you are inside your Patient Portal, you will have access to multiple resources to better access your healthcare information.
Communicate with your health care provider directly by sending and receiving secure messages through the Messages resource tab.
Access the care summaries of each visit with your provider as well as any labs discussed instantly by clicking the My Health resource tab.
Download your health record from the Task and Tools tab in a file that you can share with medical providers. Print or view referrals made by your primary care provider to other providers or care settings.
